Why IUI fails??  What is meant by timely death of a cell or programmed cell death in time?? What is enhanced apoptosis: We know that RBC dies after 120 days. We also know that there are three indications where the opinion will vary about the indication & decision to go ahead with IUI or with IVF procedure. Many gynaecologits of modern era dissatisfied with the outcome of IUI in these three following conditions will vote for IVF which is costly procedure and many Indian couple can’t afford. However, apoptosis –i,.e,. Programmed cell death (death of aged & poor quality cells of body anywhere in the body) is accelerated in these three clinical entities.

What is apoptosis?? Ans;-Apoptosis is a natural helping mechanism to get rid of the damaged cells or poor quality early embryo or any unwanted cells..  if extensive, could also lead to developmental failure. In spite of this effective program, the ability of an early embryo to undergo apoptosis appears to be developmentally regulated.,

Accelerated apoptosis like hemolysis of foetal RBC in Rh immunized mother:-:-Such abnormally high rate of cell death also affects the survival of eggs and sperms (when pushed to uterus in IUI procedure.).  These three following conditions where apoptosis is accentuated leading to even death of a good quality ova, infused sperm or fresh potentially good quality blastocyst or embryo are namely A)  minimal endometriosis, B) Untreated or incompletely treated  Pelvic  adhesive diseases & C) one sided  tube block . Additionally some cases of unexplained subfertility may have toxins which can yield to poor success of IUI!!!  In these four  (including some cases of unexplained subfertility) clinical conditions where there is preexistent enhanced apoptosis form a sizeable indications of IUI(some call it poor couples IVF)  & also a common cause of failed IUI . As I said if one separates these three cases then the success of IUI will speed up to 30-35% per cycle comparable to IVF. Let the members be aware that in endometriosis the egg quality in most cycles are persistently poor and in one sided tubal block, pelvic adhesive diseases , few unexplained subfertility  too some organism still lie in tube and or endometrium.
