What may cause Cancer Cx? Cancer of womb-is interrelated to ulcer in the mouth of womb?


 Doubts & Debates on Debate 1:-“Correlation of childbirth trauma with genesis of cervical erosion and later Ca Cx? Debate 2: How best to differentiate visually four almost similar clinical conditions like 1) cervical erosion, 2) Cx ectropion, 3) Cx cancer & 4) chlamydial Cx changes??
 Why this Question? Ans to first query?? Ans:-Bad or significant cervical erosions are seen in women who have had 1 or more normal deliveries. By contrast
Patients who have undergone caesarean sections have smooth, pink and healthy cervix. Is that your observation of members too? Does the wear and tear during delivery makes cervix more prone to erosions.--> consequently Ca cervix??  Top of Form

Secondly does caesarean protects against Ca cervix. Many clinicians have a feeling that found erosions more common with vaginitis and discharge. Erosion are not related to with LSCS or vaginal delivery as such


How to define Cx erosion clinically?? Ans:-Cervical ectropion is a condition where cells from inside the cervix (endocervical canal) form a red, inflammed patch on the outside the cervix.

What is cervical ectropion?
The cervix is the narrow part of the uterus that joins the top of the vagina.  Cervical ectropion is a condition that affects cells in the cervix. Cervix has different sorts of cells on the portio --outside (vaginal portion) than it does on the inside (the cervical canal).
The soft cells on the inside of the cervix(columnar Cells-glandular cells-not surface epithelial cells)  are known as glandular cells, but the medical term for them is columnar epithelium. By contrast , the hard cells on the outside of the cervix(portio)  are known as squamous epithelial cells. Normally in healthy women only have glandular cells on the inside of their cervix. By contrast, when the glandular cells appear on the outside of the cervix we level it as Cervical ectropion .


What do  we mean by  the transformation zone (TZ)?? The area on the outside of the cervix where glandular cells come into contact with squamous epithelial (junctional area between glandular cells) - is called the transformation zone or the stratified squamous epithelium.

How best to control PPH(postpartum hemorrhage at Cesarean delivery)?? Possible options?


Uterine atony after closure of uterus: to react or to ignore the atony at Theatre?? If we cross a busy road and see a car is coming very fast what we do we don’t take risk. Instead we wait till the high speed car crosses my way. Then and then only we cross the busy road. Uterine atony after closure of uterus is not an uncommon finding or should I say a nuisance. How best to deal with such a situation even if O T Sister affirms that there is no active bleeding PV. There are several options: Option A) No active addle step taken-just close as sister has assured U abd U have patiently waited for 5 minutes before U closed the abdomen. Option B) I V or I M methergin (methyl ergometrine) Option 3:-Intra myometrial prostaglandins Option 4:-To put Prof B Lynch Compression suture.-may be one or two. Whether to put B Lynch suture by Char catgut No 1(viral will cut through) or not?? .I had the misfortune to perform about 500 Obstet hysterectomies during my active life say 1966 to 2015 for variety reasons. Emperical uzse of nerves & amelioration of nereve pains

Good morning. Have you time to know about GABA derivatives( Pregabalin) & Gabapentin( GABA analogue):-GABA has been promoted as Neuroprotector. There are two main drugs that are used albeit empirically in neuropathic pain & mood changes in midlife of women (as anxiolytic). These two drugs are termed as namely A) Pregabalin & Gabapentin.
Former agent (Pregabalin) a GABA derivative is of little  used in our discipline. This drug is used mostly used in epilepsy & post stroke,. Initially at the dose of 50 mg OD, later 50 mg TDS. Maximum 300 mg OD.,. Brand names are Pregabid(Intas) Pregalin( Torrent) & Gabaneuron(Aristo).I have no experience of using this drug.
The second drug: GABA analogue:-But I have used second  drug i.e. Gabapentin ,This is an GABA analogue. Though like Pregabalin, this drug is also used for epilepsy but it is also used in our discipline for four  indications albeit not on regular basis or as primary drug. Such four  indications of GABAPENTIN are 1) Anxiolytic, 2) use 1:-Diabetic neuropathy ( DOSE 900-1299 mg od), Peripheral & central neuropathic pain, including pain at abd scar site when the lateral incision is cut too lateral –cut br of lateral femoral nerve is cut , use 2:- Pudendal neuropathy, use 3:- thirdly in Restless Leg syndrome-300 mg OD ,  as you watch at your chamber(such Pts are always moving their legs-like dancing their knees ?? This abnormal movement may be an expression of OCD-Obsessive Compulsive Disorders).. Use 4: Anxiolytic –may also sometimes used in Endometriosis: - Use 5 Another use  is Post-herpetic neuralgic pain when this drug Gabapentin is prescribed freely  at the dose of 300 mg TDS. The trade names of Gabapentin are Gabapin(INTAS): Gabatin, Gabapentin(ABBOTT), Gabantin(Sun Synergy)

c)   No Transfer in the same cycle: -- All Freeze –Subsequent transfer. (ER Book-Duphaston-pp.125 Ed B N Chakravarty).
d) Improve uterine vascularization.
e) To treat the existing pelvic pathological conditions:-       
f)    High E2 levels –supraphysiological levels of steroids-.This is interceptiveàAltered E2/P ratio.                                                                                                                                                                                                                                                                            
Q.1. what agents can alter receptivity of endometrium negatively? –
 A) CC & possibly antagonists prevents endometrial receptivity.
 B)  Moreover those cases where in ART cycles elevated serum progesterones in late follicular phase à ER is poorà therefore cryopreservation of embryosà transfer in next cycle is recommended. 
 C) In gonadotrophin cycles: where high steroids are circulating – may itself bock/ disfavor the implantation process. But in case of donor cycle –where recipient is not stimulated by gonadotrophins-thereby no Supraphysiological steroidsà there per cycle implantation rate is high.

Q.1. what are the different Stages of fertilized Ova before implantation? - There are basically 3 stage of nidation. Firstly embryo when enters in ut cavity at 8-16 cell stage morula. -->at cavity it becomes 32-64 cell stage, blastocyst. Q2. Prerequisites of Successful implantation:-Competent blastocyst and receptive endometrium are essential. So, Blastocyst competency and endometrial receptivity both are essential. The ideal time implantation in a 28 days cycle is day 20-24 days of cycle-called implantation window.
Query 6 :: what is the prevalence of early pregnancy loss & incidence of failed implantation?
The prevalence of early preg loss –is estimated to be 25-40%., of this 75% is due to implantation failure.

The egg cell is released by one of the ovaries and than sucked into a fallopian tube. The lining on the inside of the fallopian tube is also a highly specialized mucus membrane. The cells on the surface of this mucus membrane contain specialized hair like protrusions which is constantly moving in such a way that there is a constant movement of fluid from the ovary towards the opening of tubes and from there toward the cavity in the womb. This movement causes a suction effect. Anything in the vicinity of the ovaries ( like the egg cell) will be sucked into the womb. Refer to Normal Ovary .
An interesting fact is that fertilization does not occur in the womb , but at the opening of the tube near the ovary. That sperms swim all the way from the vagina, through the womb and upstream through the fallopian tubes.
After fertilization the fertilized egg cell is slowly sucked through the fallopian tubes into the cavity of the womb. It only arrives in the womb about five days after conception. The egg than attaches itself to the endometrium (the lining on the inside of the womb.).

We will also discuss implantation bleeding. Implantation bleeding should not be confused with bleeding during pregnancy. Why a fertile woman don't fall pregnant in each month even if she don't use any contraceptives??
