Tetanus-diphtheria toxoid (Td)
vaccine has been indicated routinely for pregnant women who have been
previously vaccinated or who have never received Td vaccine. Td vaccine protects
the mother during pregnancy and passive antibodies protects the neonate from
neonatal tetanus. .
Pregnant women should be routinely
asked when they received their last Td vaccine. If they do not know or cannot
remember then a booster dose of TdaP should be given.
Migrants labours in India, as of now,
CONVID-19 there are many Immigrants and refugees who come from other provinces
of India are infrequently fully immunized; they should receive the complete
primary series. The schedule of doses is 0, 1-2 months, 6-12 months. Frequently
the first two doses can be given during the prenatal period.
An adult formulation of the tetanus
diphtheria (Td) containing acellular pertussis antigens was licensed by the FDA
in 2005. ACIP recommended the vaccine in 2005. Since the mid 1970s the number
of cases of pertussis among adults and adolescents began to rise. This rise
was thought to be due to waning immunity in the adolescent and adult
population. Case reports suggest that morbidity from pertussis is not increased
among pregnant women as compared to non-pregnant women. However, the critical
time for pertussis infection is in the neonate in the first 6 months of life.
Hospitalizations and complications from pertussis infection are
dramatically increased in the infant less than age Since most of the disease is
in the neonate and since infant immunization occurs at 2, 4 and 6 months of age
and since the infant is not protected until 6 months of age, the logical time to immunize
mother is during pregnancy. TdaP
may be given in the second and third trimester. If immunization prior to
pregnancy or during pregnancy is missed, the vaccine should be given in the
postpartum period.
