ABC of Insulin :-Procedural defects ??Mistake 1:- Match the syringe as per vial: There
are two types of insulin syringes 40 & 100IU syringes to be used against
how many units the vial contains per ml. For instance mixtard cartridge
contains 100
units per ml whereas Mixtard U/40 from vial contains 40 units per ml . If she uses mistakenly 40 unit
syringe (instead of 100 U syringe) then the patient may ended up taking only 8 units
of insulin twice a day instead
of 20 units twice a day .
Mistake 2:- Storage of insulin if refrigerator is unavailable then keep
it under the Cot preferably dipped in
water, to keep as cool as possible ,
Mistake 3:- changing of needles at least alt days , Mistake 4:- The angle with
the needle pierces the skin, Mistake
5:- No massage after injection at the periumbilical areas(fat) ,Not at gluteal
/ thigh all these has to repeatedly counselled and supplied as a leaflet
form(pt instruction sheet), Mistake
6 Not to depend on pharmacist of nearby chemist shop who will come for
administering inj at his convenient time.
It should be stressed that Insulin
should be taken by the man/ woman concerned him /herself. Himself so that
timing remains perfect. Mistake
7 :-Take insulin and Icebox while going to vacation/ relatives house. If
stay is at hotels insist a room where refrigerator is available in the room and
while checking out see that vials are taken out. In long plane jaunty show the
Doctors prescription and get the insulin & syringe in the cabin. Mistake 8:-If having anorexia
then take 1/3 insulin less.
Pen system vs traditional syringes ? Which one better in Gynae cases, I mean in nonpreg women & men?? Ans:-
:-The pendulum is more in favour of Pen system with refill cartridges stalked
in advance . Injecting insulin through pens is less time consuming and more
convenient. In addition the dosing is precise. The upper portion of pen is
rotated in clockwise manner till the desired numerical figure of insulin dose
appears in a window at its upper end. This is known as dialing of dose. In
short, insulin pens are smarter and convenient option
. Disposable pens are preloaded with insulin cartridge inside the sealed
compartment. They are in ready to use form but once all the insulin is used up
they cannot be refilled.
Mistake 6:
Not explaining properly about hypoglycemia sym & signs and not keeping
stalk of Glucose at bedroom
Mistake 7:- Night doses a bit less. There is a
new kind of insulin (RYZODEG) which do not lead to hypoglycaemia at all So diabetologists
have opted this kind of insulin for night dose. *Ryzodeg-Refill available cost
about Rs 1900/- per cartridge).Hypoglycaemia is dangerous. If having and Keep
insulin dose at allowed dose at supper time as no one will be alert at night
time-all will sleep and in winter all doors are closed-may not listen to your
call/ cry..There may be occasionally light off at night and glucose pkt may be difficult to locate if she /he sleeps
alone,. Take care & happy reading.
Take home message: Do not take anything for granted. Do not relay on
patient . Verify each step as chest specialists insist that in every visit the spray
taking method shroud be dome and demonstrated in the presence of physician or
technician in PFT room so that right dose of bronchodilators enters into lungs alveoli. This is again like
physiotherapy.. In case of doubt(rise of blood sugar instead of Insulin) doctor
should physically inspect the vials and
syringes.
